The Transition Center is an interagency collaborative effort that includes families, schools, probation staff, public health personnel, community organizations, youth development providers, advocates, and the youth reentering the community. Our strategy for an effective juvenile justice reentry plan involves developing a coordinated continuum of multidisciplinary services guided by targeting needs identified through a validated risk/needs assessment tool. These principles represent a family-focused, trauma-informed approach that guides the overall operations of the system, i.e., the culture, the structural environment, the perspectives of providers and service delivery staff, policies and protocols, and the social and collaborative relationships. All these efforts have greatly strengthened the capacity of the Transition Center to develop “warm hand offs” and promote continuity of care.
